(There is recent evidence that a fifth species, P. knowlesi, previous thought to cause malaria only in monkeys, may also cause malaria in humans.) Malaria is transmitted by female Anopheline mosquitoes, which typically bite between dusk and dawn. The chief symptoms are fever, sweats, chills, headache, body aches, and malaise, typically accompanied by anemia and enlargement of the spleen. Complications may include jaundice, low blood sugar, kidney failure, fluid in the lungs (pulmonary edema or ARDS), and circulatory collapse. Infections caused by P. falciparum are particularly dangerous because of a propensity for infected red blood cells to obstruct blood flow to the brain, causing seizures, impaired consciousness, and sometimes coma. It is essential for all travelers to take medication to prevent malaria when visiting any area where malaria occurs, especially sub-Saharan Africa, which accounts for a majority of cases of travel-related malaria. In the United States, most cases of malaria occur in travelers who did not take appropriate malaria prophylaxis. Malaria pills should be purchased before departure, since medications obtained in developing nations may be fake. For countries where chloroquine-resistant malaria occurs, which includes most countries with malaria, there is a choice of three medications: mefloquine (Lariam) atovaquone/proguanil (Malarone)(PDF) , or doxycycline.

The Malaria Vaccine Technology Roadmap, a new global strategy, was launched December 4, 2006, at the Global Vaccine Research Forum in Bangkok. The Roadmap calls for joint action among the world's leading international health organizations to accelerate the development and licensing of a highly effective malaria vaccine. It seeks to develop a malaria vaccine by 2025 that would have a protective efficacy of more than 80% against clinical disease and would provide protection for longer than four years. The Roadmap establishes a strategic plan for developing and distributing a safe, effective and affordable vaccine to prevent malaria in children under five years of age in sub-Saharan Africa and other highly endemic regions. "The pace of progress towards a malaria vaccine could dramatically accelerate if these priority areas are successfully pursued," said Dr Melinda Moree, Director of the PATH Malaria Vaccine Initiative which coordinated the development of the Roadmap. "Information sharing and collaboration needs to be stepped up to enhance learning across studies and eliminate redundant work. Above all, continued commitment to this initiative is vital. Developing an effective malaria vaccine is an enormous challenge, but it is within reach."
