Alzheimer's disease
This factsheet is for people with Alzheimer's disease or people who would like information about Alzheimer's disease. Alzheimer's disease is rare in younger people but the risk of developing it increases with age. In the UK, about 8,000 people under 65 have Alzheimer's. About one in 10 people over 80 are affected.
What is Alzheimer's disease?
Alzheimer's disease is the most common type of dementia. Dementia is the term for the deterioration of brain function that results in loss of memory, reduced language skills, impaired reasoning and behavioural and emotional problems. An estimated 700,000 people in the UK have dementia; around 400,000 of these have Alzheimer's disease.
Symptoms
Alzheimer's disease starts with forgetfulness and difficulty in finding the right words. In these early stages of the disease, family or friends may notice changes in behaviour. For example, a previously cheerful person may become sad, withdrawn or angry. He or she may no longer be able to cope with the demands of a busy life, and become frustrated with memory loss.

    Alzheimer's disease remains a challenge in management. With nearly 8 million sufferers from this condition in the seven major markets of the world and anticipated increases in the future. Considerable research is in progress to understand the pathomechanism of the disease and find a cure. The only drugs approved currently are acetylcholinesterase inhibitors but they do not correct the basic pathology of the disease, beta amyloid deposits and neurofibrillary tangles. Several new approaches emphasize neuroprotection as well. Early diagnosis of Alzheimer's disease is an important first step in management. Several biomarkers in cerebrospinal fluid, blood and urine can detect the disease. They provide a valuable aid to the clinical examination and neuropsychological testing which are the main diagnostic methods supplemented by brain imaging. Genotyping, particularly of ApoE gene alleles is also useful in the evaluation of cases and planning management.

PET images show the metabolic degeneration of the brain of a patient with Alzheimer's as it progressively reduces brain function. In the early stages of the disease, limited areas of the brain are dysfunctional (arrows in upper image), but in late stages of Alzheimer's, larger brain areas are affected (arrows in upper and lower images). The structures that are spared and remain functioning (dark blue images) are the portions of the cortex that control the motor systems such as the legs, arms and hands (upper image), the internal structures (center structures that are dark blue in lower image) and the visual cortex at the back (bottom of the lower image). Notice that the metabolic function of the brain with late stage Alzheimer's resembles that of a child. This correlates with the child-like functional behavior of these patients. A PET scan shows a very consistent diagnostic pattern for Alzheimer's disease where certain regions of the brain have decreased metabolism early in the disease (see arrows). In fact, the pattern often can be recognized several years before a physician is able to confirm the diagnosis and is also used to differentiate Alzheimer's from other confounding types of dementia or depression.

The main characteristic of AD is memory loss[?] . Alzheimer's disease is also manifested in behavorial changes, which may even include sudden periods of defiance, abusive behavior, violence, etc. in people who have no previous history of such behavior (rarely, an affected person experiences euphoria). Thus, Alzheimer's disease presents a problem in patient management, as well. The symptoms of the disease as a distinct nosologic entity were first identified by Emil Kraepelin , and the characteristic neuropathology was first observed by Alois Alzheimer in . In this sense, the disease was co-discovered by Kraepelin and Alzheimer, who worked in Kraepelin's laboratory. Because of the overwhelming importance Kraepelin attached to finding the neuropathological basis of psychiatric disorders, Kraepelin made the generous decision that the disease would bear the name of Alzheimer (J. Psychiat. Res, 1997, Vol 31, No. 6, pp. 635-643). These changes include loss of brain tissue cells (with a typical upward progression through memory centers such as the entorhinal cortex and the hippocampus ) and collection of specific inclusions such as neurofibrillary tangles and senile plaques. It is not yet certain, whether these changes are primary (the cause of the disease) or secondary (the result of the disintegration of brain tissue). Alzheimer's disease is the most frequent reason for dementia in the elderly and affects almost half of all patients with dementia. There is no known definitive treatment, although there are drugs which enhance neutrotransmitter transmission which delay the memory loss associated with the disease.
