Abdominal Aortic Aneurysm - Treatments
Currently, there are three treatment options for AAA:
Watchful waiting - Small AAA's (less than 5 centimeters or about 2 inches), which are not rapidly growing or causing symptoms, have a low incidence of rupture and often require no treatment other than "watchful waiting" under the guidance of a vascular disease specialist. This typically includes follow-up ultrasound exams at regular intervals to determine if the aneurysm has grown.
Surgical Repair - The most common treatment for a large, unruptured aneurysm is open surgical repair by a vascular surgeon. This procedure involves an incision from just below the breastbone to the top of the pubic bone. The surgeon then clamps off the aorta, cuts open the aneurysm and sews in a graft to act as a bridge for the blood flow. The blood flow then goes through the plastic graft and no longer allows the direct pulsation pressure of the blood to further expand the weak aorta wall.
Interventional Repair - This minimally invasive technique is performed by an interventional radiologist using imaging to guide the catheter and graft inside the patient's artery. For the procedure, an incision is made in the skin at the groin through which a catheter is passed into the femoral artery and directed to the aortic aneurysm. Through the catheter, the physician passes a stent graft that is compressed into a small diameter within the catheter. The stent graft is advanced to the aneurysm, then opened, creating new walls in the blood vessel through which blood flows.

Medtronic stent may hike risk of fatal aneurysm
FDA: Patients with device more likely to die than those who had surgery
WASHINGTON - Patients treated with a Medtronic stent to prevent aneurysms appear more likely to die than those who undergo surgery, the Food and Drug Administration said Tuesday. The FDA said new data on the company’s AneuRx stent showed patients with the device were increasingly likely to die of an aneurysm. FDA issued similar alerts in 2001 and 2003, but the agency said the new data shows risks continue rising five years after the device is implanted. The small, mesh-wire tube is inserted into the abdominal aorta, a vessel that provides blood to the lower body. The stent is designed as an alternative to surgery, which is often necessary to treat dangerous bulges of the aorta. However, FDA said in rare cases the device appears to raise the risk of fatal events. FDA estimated that 1.5 percent of patients who’ve had the device five years will die from an aneurysm, well above the 0.18 percent death rate in patients undergoing surgery.

One might hear: "I'm having the worst headache of my life!" Or, "Oh, my neck is so stiff!" Treatment
Treatment Almost all brain aneurysms need to be treated. If the aneurysm has already ruptured, the ultimate goal of treatment is to prevent another rupture and future bleeding while preserving the artery from which the aneurysm originated. If the aneurysm has not yet ruptured, the goal is to prevent it from doing so. After a ruptured aneurysm is diagnosed, the aneurysm is secured as quickly as possible using a variety of surgical or endovascular coiling techniques. Surgery
  • Microsurgical Clipping The majority of aneurysms can be successfully treated using a surgical technique called microsurgical clipping. Aneurysms are located in the area outside the brain, called the subarachnoid spaces and fissures. During microsurgical clipping, the aneurysms are accessed by carefully opening these areas under the high magnification of operating microscopes. The surgeon then spreads the brain tissue apart and places small metal clips at the base of the aneurysm at the point where it arises from the artery, while maintaining this artery so that a stroke does not occur. After clipping the aneurysm, the bone is secured in its original place, and the wound is closed. Skull Base Surgery This surgery is typically used for deep and complex aneurysms, which are accessed through the bone at the base of the skull.

An aneurysm is a swelling or dilation of a weakened wall in an artery. Arterial aneurysms have a tendency to grow and burst causing life threatening bleeding. Treatment of this disease has been ongoing for many years through open surgical repair of the dilated artery. Using an abdominal incision, open surgical repair of abdominal aortic aneurysms allows complete removal of the aneurysm with replacement using a piece of Teflon tubing. Within the last five years a new approach to treating aortic aneurysms has been developed. Summary of Recommendation
  • The USPSTF recommends one-time screening for abdominal aortic aneurysm (AAA) by ultrasonography in men aged 65 to 75 who have ever smoked. Rating: B Recommendation Rationale : The USPSTF found good evidence that screening for AAA and surgical repair of large AAAs (5.5 cm or more) in men aged 65 to 75 who have ever smoked (current and former smokers) leads to decreased AAA-specific mortality. There is good evidence that abdominal ultrasonography, performed in a setting with adequate quality assurance (i.e., in an accredited facility with credentialed technologists), is an accurate screening test for AAA. There is also good evidence of important harms of screening and early treatment, including an increased number of surgeries with associated clinically-significant morbidity and mortality, and short-term psychological harms. Based on the moderate magnitude of net benefit, the USPSTF concluded that the benefits of screening for AAA in men aged 65 to 75 who have ever smoked outweigh the harms. The USPSTF makes no recommendation for or against screening for AAA in men aged 65 to 75 who have never smoked.
