A 43-year-old man presented with acute-onset hemianesthesia involving the right arm, the right leg, and the right side of the face. Six weeks earlier, he had a moderate occipital headache that lasted for 3 days. Diffusion-weighted magnetic resonance imaging (MRI) of the brain showed a thalamic lesion of restricted diffusion on the left side (Panel A, arrow), which was consistent with acute posterior choroidal-artery infarction. Abdominal Aortic Aneurysm An aneurysm is another type of disease that affects the arteries. It is a localized widening or enlargement of an artery compared to its normal size. An aneurysm is a serious health problem because it may rupture, similar to the bursting of a balloon that has been over inflated. When a blood vessel with an aneurysm ruptures, life-threatening bleeding occurs. Aneurysms can also cause pain from pressure on nearby organs or nerves. Occasionally, debris ( emboli ) contained within the aneurysm can break off and travel to the legs or vital organs, blocking the blood flow to these tissues. Aneurysms occur most commonly in the abdominal aorta, a large blood vessel in the abdomen. Abdominal aortic aneurysms ( AAA's ) most often involve the infrarenal aorta , or that portion of the blood vessel that lies below the takeoff of the arteries to the kidneys ( renal arteries ). About half of AAA's also involve the iliac arteries in the pelvis. Uncommonly, AAA's extend above the renal arteries (suprarenal) and possibly into the chest cavity ( thoraco-abdominal AAA's affect 6-9% of men over 65 years of age. They are twice as common in men than in women. The major risk associated with AAA's is that they have a high propensity to rupture. Ruptured AAA's are the 13th leading cause of death in the United States. They are also the 10th leading cause of death for men over the age of 55. Approximately 30% to 50% of patients with a ruptured AAA die before they ever reach a hospital. Even with surgery, there is 50-70% mortality rate associated with a ruptured AAA. Therefore, early detection and timely repair are paramount to AAA management.

A retrospective analysis of 100 patients suffering from a ruptured brain aneurysm has found that expensive new coiling devices are no more effective than bare platinum coils at preventing aneurysms from recurring.
A team of researchers from the Neuroscience Institute at the University of Cincinnati (UC) and University Hospital, led by Andrew Ringer, MD, evaluated the risk of intracranial aneurysm recurrence after endovascular treatment with each of three different kinds of coils.
Ringer, the study's principal investigator, is a neurosurgeon with the Mayfield Clinic and is director of endovascular neurosurgery and associate professor of neurosurgery at UC.

Aortic Aneurysm
Overview
Mayo Clinic physicians and surgeons are internationally recognized experts in the treatment of thoracic and abdominal aortic disease. Mayo Clinic is among the highest-volume centers treating these conditions, repairing more than 500 aortic aneurysms each year, with excellent results. Patients are referred to Mayo Clinic from all over the world to benefit from Mayo's expertise in treating challenging aneurysms, such as those involving the aortic root (where the aorta emerges from the heart) and the aortic arch, and those involving both the chest and abdomen. Surgeons at Mayo Clinic helped develop minimally invasive endovascular surgery techniques that shorten recovery time and hospital stays for patients. These methods offer a viable option for many patients who have chronic illnesses and are at risk for surgical complications. Mayo vascular, cardiac and thoracic surgeons work with cardiologists and other medical specialists as needed to plan care for each patient. In Mayo's efficient system, testing and treatment can usually be completed in days, not months.

University of Michigan Health System, Ann Arbor, Michigan Patient information: A handout on abdominal aortic aneurysms, written by Jill Giordano, Medical Editing Clerk at Georgetown University Medical Center, is provided on page 1205. See accompanying editorial on page 1157. A PDF version of this document is available. Download PDF now (7 pages /167 KB). A bdominal aortic aneurysm (AAA) is a relatively common and often fatal condition that primarily affects older patients. AAAs and aortic dissections are responsible for at least 15,000 deaths yearly and in 2000 were the 10th leading cause of death in white men 65 to 74 years of age in the United States. With an aging population, the incidence and prevalence of AAA is certain to rise. Most AAAs are asymptomatic, and physical examination lacks sensitivity for detecting an aneurysm. It is important that family physicians understand which patients are at risk for the development of AAA and the appropriate evaluation once a patient has been diagnosed with an aneurysm.

Posts of the Year, 2006
This year represented my third full calendar year of blogging. It was also the year I suspended the Daily Aneurysm in favor of doing all my current-events blogging at Best of the Blogs because the traffic is higher there. The switch to BotB cost us the contributions of Tom Herbst, who posted occasionally in this space for about a year. We haven't heard from Tom, either via e-mail or in the comments, for a good long while, but I'm hoping he's still out there somewhere, flying his liberal flag in Pennsylvania, where it's needed: After all, Pennsylvania is a place James Carville once described as Pittsburgh on one end and Philadelphia on the other with Alabama in the middle. A couple of Tom's nuggets of wisdom appear on this list of my favorite posts of 2006:
January 12:
"We may have reached a new low in American dumbitude, given that the big story to come out of the Alito hearings yesterday was that the mean Democrats made Mrs. Alito cry. Except they didn't. It was South Carolina Repug Senator Lindsey Graham who did, by repeating something a Democrat had said about her husband, or some such damn thing. (I confess I haven't spent a lot of time investigating this story on my own because I am afraid that if I get too close to it, I will get stupidity cooties.)"

Replay of Endovascular Aneurysm Repair
A procedure to treat patients with abdominal aortic aneurysm (AAA) March 18, 2008
6:00 PM CDT
(March 19, 2008 at 00:00 UTC) From Our Lady of the Lake Regional Medical Center, Baton Rouge, LA Baton Rouge, LA – Our Lady of the Lake Regional Medical Center will broadcast a minimally invasive surgery to repair an aortic aneurysm live over the Internet on Tuesday, March 18th, 6:00 pm central time. An abdominal aortic aneurysm (AAA) is a bulge in the aorta which could rupture with life-threatening results. Although an aneurysm can occur in any artery of the body, it is most common in the abdominal aorta. This condition, if not treated, could result in a rupture of the aorta. The risk of rupture increases with aneurysm size and high blood pressure. Ruptured aneurysms are frequently fatal and are a leading cause of death in the United States. Many people do not experience any symptoms of a AAA. Because of this, it is very important to speak with your doctor about your risk of having or developing AAA disease. When symptoms do occur, pain is most commonly experienced. This can occur in the abdomen, back or chest area. Aneurysms are found during medical tests such as a CT Scan or ultrasound.

Abdominal Aortic Aneurysm (AAA) Screening
Abdominal Aortic Aneurysm (AAA) is a vascular disease with life-threatening implications. If you have a family history of abdominal aortic aneurysm or have smoked at least 100 cigarettes in your lifetime, you are considered at risk.
How often is it covered?
Medicare covers this one-time screening ultrasound if you get a referral for it as a result of your "Welcome to Medicare" physical exam. You must receive the physical exam and the screening ultrasound referral (not the ultrasound exam itself) within the first six months you have Medicare Part B.
For whom?
People with Medicare who meet the following criteria are eligible:
  • He or she must get a referral for the AAA ultrasound screening from a physician or other qualified non-physician practitioner as a result of their "Welcome to Medicare" physical exam.
