Adult and Pediatric Outpatient Neurorehabilitation
Brain injuries affect thousands of children and adults each year. They may be the result of trauma to the brain from car accidents, falls, strokes, tumors, infections, and seizure disorders. These injuries can result in impairments of cognitive and physical functions, as well as changes in personality and behavior. In turn, these problems may affect academic and work performances. For the family of an individual with a brain injury, this can create stress and disrupt the family system. Brain injuries are complex problems that require intensive rehabilitative efforts during both hospital and post-hospital phases. Our adult and pediatric programs in neurobehavioral rehabilitation provide a variety of comprehensive inpatient/outpatient programs for adults and children with brain injuries. Each program is individualized and is provided by our interdisciplinary team of brain injury specialists.
Outpatient Rehabilitation
A variety of outpatient services are provided for adults and children with brain injuries. Return visits to our Outpatient Rehabilitation Services allow the treatment team to assess the individual's progress and to monitor and update his or her post-hospital rehabilitation plan. Individuals suspected of having a brain injury, as well as those who require a comprehensive evaluation, rehabilitation program or assistance with community/work/school re-entry, may also be appropriate for referral to our outpatient services.

      Jim was only 24 years old when he fell two stories at work and sustained a severe brain injury. In addition to his brain injury, Jim suffered multiple fractures and internal injuries as a result of his fall. After he woke up from his coma, his doctor referred him to Bethesda Hospital. Jim had a private room on the Brain Injury Rehabilitation 14-bed unit. His wife was encouraged to bring family pictures and other items to stimulate his memory. Jim had therapy twice a day, six days a week to restore his strength and endurance. Jim had to relearn how to walk, talk and dress himself.

What are "acquired brain injury" and "traumatic brain injury" and how do they differ?
Traumatic brain injury (TBI) includes any injury to the structure of the brain caused by an external force or insult. Acquired brain injury (ABI) is defined as damage to the brain that occurs after birth and is not the result of a congenital defect or a progressive degenerative disease. ABI may be the result of physical trauma or various medical conditions
How are brain injuries classified?
There are mild, moderate and severe head injuries. A person sustaining a mild head injury may or may not lose consciousness, but may experience some symptoms, even days after the injury which often go untreated. Moderate head injury may be characterized by being unconscious less than 24 hours and may require rehabilitation. Severe head injury survivors usually experience a coma, require rehabilitation, and often need outpatient care and/or long term care.

The brain is a three-pound mass of gray matter enclosed in the cranium. It interprets sensory impulses, coordinates and controls bodily functioning and is the center of emotion and thought. The brain has three main sections: the cerebral hemispheres, cerebellum and brain stem. There are left and right cerebral hemispheres. They are divided into sections called lobes. The left cerebral hemisphere controls the right side of the body and is responsible for speech. The right cerebral hemisphere controls the left side of the body and is responsible for creative thinking. It’s estimated that 8 million people sustain brain injuries each year in the United States. At least 2 million of those injured will be permanently impaired. After brain injury things that once were easy and familiar become strange and difficult. However, many of these minor head injuries have significant, long-lasting results. In many cases, the injuries which result in traumatic brain injury may be bloodless, require no medical care, and initially seem non-disabling. In fact, many times the victim does not lose consciousness at the accident scene. Additionally, in many cases, physical and neurological examination, x-ray, CAT scan, and MRI scan will reveal no objective problems, but microscopic research has shown that irreversible nerve damage can occur even when the head does not strike an object, but instead is only shaken violently as in a whiplash incident. Traumatic brain injury cases require highly technical evaluation and extensive attention to detail. This is because many health care providers as well as attorneys often misunderstand and misevaluate the potential devastation of a mild brain injury. Many of the symptoms of mild to moderate organic brain injury are subtle, making it difficult to assess; and often a client's initial treating or emergency room physician will fail to detect traumatic brain injury. Moreover, a client with traumatic brain injury is often unaware or denies that she suffers from such an injury despite obvious changes in her personality, work habits, memory or lifestyle. Consequently, these victims never receive proper medical attention for the physical, cognitive, and psychological impairment that follows.

Traumatic Brain Injury
Traumatic brain injury is an insult to the brain caused by an external physical force, that may produce a diminished or altered state of consciousness, which results in an impairment of cognitive abilities or physical functioning. It can also result in the disturbance of behavioral or emotional functioning. These impairments may be either temporary or permanent and cause partial or total functional disability or psychosocial maladjustment.
Traumatic Brain Injury can be cause by shaking (as in Shaken Baby Syndrome), a direct blow to the head (concussion), or impact from bullet, knife or other sharp object that forces hair, skin, bone and fragments from the object into the brain (penetration injury).
Closed Head Brain Injury
When a person receives an impact to the head from an outside force, but the skull does not fracture or displace this condition is termed a "closed head injury". The brain swells but has no place to expand, thus causing increased pressure within the skull which can force brain tissues to compress, causing further injury.
When a person suffers severe physical injuries, the focus is often on the visible injuries. Closed head injury to the brain is one of the most overlooked and sinister outcomes of traumatic injuries. Broken bones and cuts can heal, but the unseen injury of a bruised brain may never fully recover.
