Who Should Get the HPV Vaccine?

Medical Editor: William C. Shiel Jr., MD, FACP, FACR Human papillomavirus (HPV) infection of the cervix can lead to cervical cancer. A vaccine designed to prevent cervical cancer and other diseases caused by infection with HPVs was approved for use in the U.S. in June 2006. This is the first vaccine to be developed against a known risk factor for the development of a cancer While some HPV types infect the skin and cause benign warts men and women The most common HPV types that infect the genital area are HPV types 6, 11, 16, and 18. Among these, HPV types 6 and 11 are most commonly associated with benign lesions, such as genital warts and mild precancerous changes of the cervix. In contrast, HPV types 16 and 18 are the types found in the majority of cancers as well as in severe precancerous changes of the cervix. The uterus also has a round leiomyoma up higher. Cervical cancer malignant cancer of the cervix uteri or cervical area. It may present with vaginal bleeding but symptoms may be absent until the cancer is in its advanced stages. Treatment consists of surgery (including local excision ) in early stages and chemotherapy and radiotherapy in advanced stages of the disease. Pap smear screening can identify potentially precancerous changes. Treatment of high grade changes can prevent the development of cancer. In developed countries, the widespread use of cervical screening programs has reduced the incidence of invasive cervical cancer by 50% or more. Human papillomavirus (HPV) infection is a necessary factor in the development of nearly all cases of cervical cancer.

Description of the Disease The cervix is the lower, narrow part of the uterus (womb) that serves as the birth canal. Like all organs of the body, it is made up of different types of cells. The various cancers of the cervix are named for the type of cell in which they originate. About 85 to 90 percent are squamous cell carcinomas. Squamous cells are thin, flat cells that form the surface of the cervix. The remaining 10 to 15 percent are typically adenocarcinomas, which start in the mucous-producing gland cells. If the cancer has features of both types it is called mixed (or adenosquamous) carcinoma. Back to Top Who is at Risk? Each year about 11,150 American women will learn they have invasive cervical cancer, and about 3,670 will die from this disease. Cervical cancer occurs most often in women between age 35 and 55. Non-invasive cervical cancer, where the abnormal cells are limited to the surface of the cervix, is about four times as common as invasive cervical cancer. Cervical cancer is termed invasive when the abnormal cells are found deeper in the cervix or have spread beyond the cervix into other organs or tissues. When found and treated early, cervical cancer often can be cured.

We are counting on you to help do that ... and to eliminate cervical cancer in the U.S. Cervical cancer is the only cancer for which we know the cause virtually all cervical cancers (99%) are caused by the human papillomavirus (HPV), which can be detected through regular screenings BEFORE IT BECOMES CANCER! Yet, in 2006, more than 9,700 women in the U.S. will be diagnosed with cervical cancer and more than 3,700 will die from it.     Pap with HPV DNA Testing
    Pap with high-risk HPV testing is a recommended screening approach for women age 30 and older. With this test, women receive both a cervical cytology test and a genetic test that looks for certain high-risk types of HPV known to cause cancer (Pap with HPV DNA test). When using Pap testing with HPV DNA, the combined sensitivity for detecting high-grade cervical disease and cancer has been reported as more than 99%.
    By using the Pap with HPV DNA as a combined primary screening method for women age 30 and older, physicians are able to:
    • Identify women who are at greatest risk for cervical disease or cancer Identify women who may have a persistent infection that can be about 300 times more likely to develop HSIL Offer appropriate and effective follow-up, since the combined Pap with HPV DNA test is more sensitive in determining the presence of disease than the Pap test alone.
