The Hepatitis B Virus (HBV)
HBV is a mostly double-stranded DNA virus in the Hepadnaviridae family. HBV causes hepatitis in human and related virus in this family cause hepatitis in ducks, ground squirrels and woodchucks. The HBV genome has four genes: pol, env, pre-core and X that respectively encode the viral DNA-polymerase, envelope protein, pre-core protein (which is processed to viral capsid) and protein X. The function of protein X is not clear but it may be involved in the activation of host cell genes and the development of cancer.
Risk Factors for HBV Infection
Although relatively rare in the United States, hepatitis B is endemic in parts of Asia where hundreds of millions of individuals may be infected. HBV is transmitted horizontally by blood and blood products and sexual transmission. It is also transmitted vertically from mother to infant in the perinatal period which is a major mode of transmission in regions where hepatitis B is endemic. The blood supply in developed countries has been screened for HBV for many years and at present transmission by blood transfusion is extremely rare. Major routes of transmission among adults in Western countries are intravenous drug use and sexual contact. The risk of HBV infection is notably high in promiscuous homosexual men but it is also transmitted sexually from men to women and women to men. Transmission is probably prevented by correct use of condoms. Health care workers and patients receiving hemodialysis are also at increased risk of infection.

Hepatitis C is a disease of the liver that is caused by the hepatitis C virus. The disease occurs in acute and chronic forms; symptoms can range from mild (or even no symptoms) to severe. Hello, and welcome to " A Hepatitis C Prime r ". I set up this web page as a starting point for anyone who has contracted the Hepatitis C virus (HCV), or for anyone wanting to learn more about Hep C. Although I wasn't diagnosed with HCV until mid 1992, I believe I contracted the virus either in Nov 1977 or Jan 1980 from blood transfusions I was given during the birth of my children. So that means I have been infected at least 20+ years. While I don't claim to know all there is to know about HCV, I have read every scrap of research I can find in my quest for knowledge about this 'silent killer' and have tried to incorporate my findings on this site. I've tried to include a little bit of everything here on a Hepatitis C Primer and hope that all the visitors here will enjoy their stay, and find answers to some of their questions about HCV. You'll find the latest research reports as well as some older research reports I keep on my site for those digging up background information about the disease. HEPATITIS C What is hepatitis? Hepatitis is an inflammation of the liver caused by certain viruses and other factors, such as alcohol abuse, some medications and trauma. Its various forms affect millions of Americans. Although many cases of hepatitis are not a serious threat to health, infection with certain hepatitis viruses can become chronic (long-lasting) and can sometimes lead to liver failure and death. How many kinds of viral hepatitis are there? There are four major types of viral hepatitis, all caused by different viruses: hepatitis A, hepatitis B, hepatitis C and delta hepatitis. This "HealthBeat" focuses on hepatitis C. What is hepatitis C and how is it transmitted? This type of hepatitis is caused by the hepatitis C virus (HCV), which infects approximately 36,000 persons each year in the United States. Although generally a mild condition, hepatitis C is much more likely than hepatitis B to lead to chronic liver disease. People infected with HCV can become chronic carriers of the virus, although they may have no symptoms. It is estimated that there are approximately 4 million HCV chronic carriers. Around 70 percent of all HCV carriers will develop chronic liver disease, regardless of whether they have symptoms. HCV is spread primarily by exposure to human blood. Approximately 80 percent of persons who share needles to inject drugs are infected with HCV. Persons who receive blood transfusions face some risk, although it is very low since testing of donated blood for HCV began in 1990. Hepatitis C has been transmitted between sex partners and among household members, but the degree of risk is believed to be low. HCV is not spread by food or water or casual contact, such as shaking hands or sharing a work space or bathroom facility.

We can cure about half of the patients with chronic hepatitis C, including some with advanced cirrhosis and liver failure. If you are cured, your viral level falls to undetectable during treatment and stays undetectable the rest of your life, after you stop the medications. Some patients who are clinically cured of HCV may still have tiny amounts of HCV-RNA , but the implications of this are not yet clear. If your HCV-RNA level 6 months after your last injection of interferon, you have a 97-99% chance of permanent cure. I have only five patients out of many hundreds who turned HCV-RNA positive later than 6 months after their last injection of interferon one year after completion of treatment. Four turned positive one year after the last injection, and one turned positive 18 months after the last injection.
