Hiatus Hernia The hiatus hernia is one of the most misunderstood and maligned conditions in medicine. People blame this hernia for much more than it ever does. Patients with a hiatus hernia need to understand what it is and what might occur with it. Most importantly, they need to know it is unusual for serious problems to develop from this type of hernia. Anatomy
The diaphragm is a sheet of muscle that separates the lungs from the abdomen. When a person takes a deep breath, the dome-shaped diaphragm contracts and flattens. In doing this, the diaphragm pulls air into the lungs. The left diaphragm contains a small hole through which passes the tube-shaped esophagus that carries food and liquid to the stomach. Normally this hole, called a hiatus, is small and fits snugly around the esophagus. The J-shaped stomach sits below the diaphragm. What Causes a Hiatus Hernia?
In some people, the hiatus or hole in the diaphragm weakens and enlarges. It is not known why this occurs. It may be due to heredity, while in others it may be caused by obesity, exercises such as weightlifting, or straining at stool. Whatever the cause, a portion of the stomach herniates, or moves up, into the chest cavity through this enlarged hole. A hiatus hernia is now present. Hiatus hernias are very common, occuring in up to 60 percent of people by age 60.

    • The sort of person likely to get a hernia is not necessarily the one who lifts a heavy weight badly. A hernia is just as likely to occur during exercise, coughing, straining on the toilet or even as a result of having a lively sex life. Hernias occur because of muscle weakness, often in or around the abdomen. Hernias happen where the cavity wall is weak. These points of weakness occur where there is a natural gap such as where a digestive tube or blood vessel passes or because of scar tissue.

    Hernia is a general term for a bulge or protrusion of an organ through a part of the body in which it is usually contained.
    DESCRIPTION
    There are many different kinds of hernias. The most familiar are those that occur in the abdomen. In this type of hernia, a part of the intestines protrudes (sticks out) through the wall of the abdomen. An abdominal hernia can occur in different areas. The name given to the hernia depends on the location in which it occurs. Some examples of abdominal hernias are the following:
    • An inguinal (pronounced IN-gwin-null) hernia appears in the groin. It may come and go depending on various factors, such as the amount of physical activity. Inguinal hernias account for 80 percent of all hernias. They are more common in men. Femoral (pronounced FEH-muh-rull) hernias are similar to inguinal hernias, but they occur lower in the body. They are more common in women, and commonly occur during pregnancy. A ventral hernia is also called an incisional hernia. The name reflects the fact that it often occurs at the location of an old surgical scar (incision). A ventral hernia is caused by the stretching of scar tissue. It occurs most commonly in pregnant women and people who are obese (excessively over-weight, see obesity entry).

    What is a hernia?
    A hernia occurs when a section of intestine protrudes through a weakness in the abdominal muscles. A soft bulge is seen underneath the skin where the hernia has occurred. In children, a hernia usually occurs in one of two places:
    • around the belly-button in the groin area
    A hernia that occurs in the belly-button area is called an umbilical hernia
    A hernia that occurs in the groin area is called an inguinal hernia
    What causes a hernia?
    A hernia can develop in the first few months after the baby is born because of a weakness in the muscles of the abdomen. Inguinal and umbilical hernias happen for slightly different reasons.
    What is an inguinal hernia?
    As a male fetus grows and matures during pregnancy, the testicles develop in the abdomen and then move down into the scrotum through an area called the inguinal canal. Shortly after the baby is born, the inguinal canal closes, preventing the testicles from moving back into the abdomen. If this area does not close off completely, a loop of intestine can move into the inguinal canal through the weakened area of the lower abdominal wall, causing a hernia. Although girls do not have testicles, they do have an inguinal canal, so they can develop hernias in this area as well.

    What is a sports hernia?
    A sports hernia, also known as athletic pubalgia, is a tear in the muscles of the lower abdomen. It causes pain in the lower abdomen or groin. Men get athletic pubalgia more commonly than women.
    How does it occur?
    The pain from a sports hernia usually begins slowly. There are different theories about how this hernia occurs. One possibility is that tight or strong hip flexor muscles tilt the pelvis forward and stretch the lower abdominal wall muscles, eventually leading to small tears in the muscles and tissues. Activities that require a lot of bending over and leaning forward can lead to this problem. Athletes who play soccer, ice hockey, rugby, field hockey, tennis, or run track are most likely to get a sports hernia.

    What is a diaphragmatic hernia?
    Congenital diaphragmatic hernia (also known as CDH) is an abnormality that occurs before birth as a fetus is forming in the mother's uterus. The specific defect is an opening or whole in the diaphragm (the muscle that separates the chest cavity from the abdominal cavity). With this defect, some of the organs that normally are found in the abdomen move up into the chest cavity through this abnormal opening.
    What causes a diaphragmatic hernia?
    As a fetus is growing before birth, different organ systems are developing and maturing. The diaphragm normally forms between the 7th and 10th weeks of pregnancy. The esophagus (tube that leads from the throat to the stomach), the stomach, and the intestines are also developing at this time. For reasons that are unclear, the muscle in babies with CDH does not form completely. This abnormality is most commonly seen on the left side of the body but may also occur on the right side or the central portion of the diaphragm. CDH involves many factors, both genetic and environmental. It is thought that multiple genes from both parents, as well as a number of environmental factors that scientists do not yet fully understand, contribute to its presence.
