Hernia
Hernias occur when part of the internal tissue (fat, muscle, intestines etc) bulge through a weakness in the overlying abdominal wall. The herniating tissues are at risk of complications such as strangulation (pinching) which can cause intestinal blockages and may cut off the blood supply which requires immediate treatment. There are 3 common forms of hernia:
Inguinal hernia
This is a protrusion of the contents of the abdomen (belly) through the inguinal canal. It usually appears as a swelling in the groin area. Eighty percent of all hernias are inguinal, men are 25 times more likely to suffer an inguinal hernia and 5% of the population will suffer an inguinal hernia. Inguinal hernias can either be direct or indirect. Direct hernias happen when part of the abdomen herniates through a weak point in the abdominal wall and into the inguinal canal. Indirect hernias occur when part of the abdomen bulges through the deep inguinal ring (this is less common and occurs due to a birth defect. Symptoms include:
  • Pain in the groin when you exercise Pain on increases in abdominal pressure such as when coughing or sneezing.

The danger from herniation arises when the organ protruding through the opening is constricted to the extent that circulation is stopped or when the protruding organ impairs the function of other structures. What is an inguinal hernia? Inguinal hernias account for about 80% of all hernias and are the most common surgical procedures done in infancy. These hernias appear more frequently in boys than in girls. An inguinal hernia is derived from persistence of all or part of the processus vaginalis, the tube of peritoneum that precedes the testicle into the scrotum during the eighth month of pregnancy. Following the dropping of the testicles into the scrotum, the processus vaginalis withers and closes forming the tunica vaginalis that lies below the testicles in the scrotum. When this fails to happen, fluid from the abdomen or an abdominal organ (usually the intestines) can be forced into it causing a bulging or mass that can be felt. The process vaginalis can extend only partly from the inguinal canal or extend completely into the scrotum. Normal scrotum:
the processus vaginalis and tunica vaginalis are obliterated and contain no fluid or abdominal contents Inguinal hernia:
the processus vaginalis has remained open allowing abdominal contents (fluid and loops of bowel) to enter into the scrotum

The FDA issued an immediate recall of Bard Composix® Kugel Mesh Hernia Repair Patches on December 22, 2005 and updated the list again on January 10, 2007 (See the Recall Notices and Affected Device List Below) The "memory recoil ring" that opens the Composix Kugel Mesh Patch after it has been inserted into the intra-abdominal space can break. This can lead to bowel perforations and/or chronic intestinal fistulae (abnormal connections or passageways between the intestines and other organs). The FDA has advised patients who have received this device to seek medical attention immediately if abdominal pain, fever, tenderness at the implant site or other unusual symptoms occur. The FDA has instructed surgeons and hospitals to immediately stop using the recalled products and return the unused patches to the company.

Hiatal Hernia
  • What Is It? Symptoms Diagnosis Expected Duration ... Additional Info
  • What Is It? A hernia occurs when part of an internal organ or body part protrudes through an opening into an area where it shouldn't. A hiatal hernia is named for the hiatus, an opening in the diaphragm between your chest and your stomach. Normally, the esophagus (the tube that carries food to the stomach) goes through this opening. In a hiatal hernia, part of the stomach and/or the section where the stomach joins the esophagus (called the gastroesophageal junction) slips through the hiatus into the chest.

Though diaphragmatic hernia may occur as an isolated defect that is potentially repairable, most are associated with multiple anomalies, and often with chromosomal abnormalities.

A hernia is a protrusion of an organ or tissue out of the body cavity in which it normally lies. By far the most common hernias develop in the abdomen, when a weakness in the abdominal wall evolves into a localized hole, or "defect", through which the protrusion occurs. These may present either with pain at the site, a visible or palpable lump, or in some cases by more vague symptoms resulting from pressure on an intraabdominal organ which has become "stuck" in the hernia. Fatty tissue usually enters a hernia first, but it may be followed by or accompanied by a segment of intestine or other intraabdominal organ. It is generally advisable to repair hernias in a timely fashion, in order to prevent complications which include intestinal blockage, gangrene, and death. Most hernias can be surgically repaired. Recovery rarely requires long-term changes in lifestyle. A hernia may be likened to a failure in the sidewall of a pneumatic tire. The tire's inner tube behaves like the organ and the sidewall like the body cavity wall providing the restraint. A weakness in the sidewall allows a bulge to develop, which can become a split, allowing the inner tube to protrude, and leading to the eventual failure of the tire.

        Definition:
Hiatal hernia repair is surgery to correct a defect in the diaphragm, the breathing muscle that separates the chest area from the stomach area.
        Alternative Names:
        Fundoplication; Anti-reflux surgery
        Description:
A hiatal hernia occurs when the normal opening in the diaphragm is too large. If the problem is not repaired, the stomach or other abdominal contents may bulge (herniate) into the chest, causing heartburn (acid reflux) and serious damage to the esophagus. In some cases, a hiatal hernia can cause twisting of the intestines or stomach. This condition is called a voluvlus. It is a life-threatening emergency that requires immediate treatment. For an open hiatal hernia repair, a cut is made in the stomach area (abdomen) while the patient is under general anesthesia (asleep, no pain). The surgeon tightens the hiatus. The upper part of the stomach (fundus) may be wrapped around the esophagus (fundoplication) to reduce heartburn due to acid reflux. Sometimes the surgeon will place a temporary tube from the stomach through the abdominal wall to keep the stomach in place.

LUCIE Moises Alou has been stricken with the latest - and most severe - injury to a Met. The 41-year-old left fielder was diagnosed Wednesday with a hernia and will be operated on today by Dr. Eugene Nowak at New York Presbyterian. Team spokesman Jay Horwitz estimated a four- to six-week absence before baseball activities can resume, suggesting Alou won't be ready for major-league games until at least mid-May. The devastating news leaves the Mets dangerously thin in the outfield, and in terms of providing protection for No. 5 hitter Carlos Delgado . Barring an outside acquisition, Damion Easley Marlon Anderson and Endy Chavez - all banged up right now - as well as Angel Pagan will be asked to offset Alou's lost power numbers and patrol left field.

    What is a hiatal hernia?
A hernia is a protrusion of part of an organ through the muscle wall that surrounds it. A hiatal hernia occurs when the upper part of the stomach pushes up into the chest through a small opening in the diaphragm, the muscle that separates the stomach from the chest. A hiatal hernia results in retention of acid and other contents above this opening. These acids and other substances can easily back up (reflux) into the esophagus. Many people age 50 and over have small hiatal hernias; however, hiatal hernias may affect people of all ages.
    What causes hiatal hernia?
Suggested causes of hiatal hernia include the following:
    • coughing vomiting straining sudden physical exertion
Obesity and pregnancy can also contribute to this disorder.
    Treatment for hiatal hernia:
Specific treatment for hiatal hernia will be determined by your physician based on:
    • your age, overall health, and medical history extent of the condition your tolerance for specific medications, procedures, or therapies

Each year about 600,000 hernia-repair operations are performed in the United States. Until recently, however, all were performed as traditional, "open" procedures requiring a large incision in the lower abdomen. The result was significant pain for patients.
Today, the minimally invasive technique of laparoscopic surgery can be used to repair the most common types of hernias. Although both traditional and laparoscopic hernia surgery are performed on an outpatient basis, patients treated laparoscopically seem to experience more rapid healing and far less pain during recovery. They can return to normal activity, including work, after only a few days, while recovery from traditional hernia repair can be a three- to five week process.
The laparoscopic approach can be used to repair direct and indirect inguinal (groin) hernias and femoral (below the groin) hernias. Some hernias located in other parts of the abdominal wall can be repaired laparoscopically as well.

CAN IT CAUSE SHORTNESS OF BREATH Dear Reader, sliding hernia is when the stomach slides up into the chest and back down; a fixed hernia is when the stomach slides up into the chest and stays there. Both can be quite painful conditions, although some minor hiatal hernias can cause mild to no pain. Because the stomach assumes space in the chest, leaving less room for the lungs to function at capacity, yes, hiatal hernias can cause shortness of breath. Hiatal hernias can also cause:
    • chest pain
    • heartburn
    • belching
    • difficulty swallowing
Shortness of breath and pain around the breastbone area also can be caused by a hiatal hernia, but can stem from heart problems, as well. If you experience any of these symptoms, it's a good idea to check in with a health care provider to make sure a more severe underlying condition isn't at work. Other warning signs that require medical evaluation include dizziness, irregular heartbeat, and nausea or vomiting. In order to diagnose a hiatal hernia, a health care provider may perform a simple chest X-ray, possibly with a
