Leprosy is a bacterial disease of the skin and nervous system caused by Mycobacterium leprae. The two main types of leprosy are called lepromatous and tuberculoid. Tuberculoid leprosy is not related to tuberculosis. Who gets leprosy?
Leprosy is usually found in South Asia, Southeast Asia, tropical Africa and some areas of Latin America. Although it is occasionally found in the southern part of the United States, most persons diagnosed with leprosy in this country are immigrants or refugees who were infected in their native countries. What are the symptoms of leprosy?
In lepromatous leprosy, the skin lesions are many and widespread. Infection in the nose may cause crusting, bleeding and blockage; infection in the eye may cause inflammation of the iris and cornea. In tuberculoid leprosy, the skin lesions are fewer and have little or no feeling. There may also be numbness in the hands, feet or other parts of the body. Lepromatous leprosy is much more contagious than tuberculoid leprosy. How soon after exposure do symptoms appear?

Victorian statutory requirement
Leprosy (Group B disease) must be notified in writing within five days of diagnosis. School exclusion: exclude until approval to return has been given by the Secretary. Contacts are not excluded.

Mycobacterium leprae is the causative agent.
Identification
Clinical features
Leprosy is a slowly progressive bacterial infection involving the cooler body tissues, skin, superficial nerves, nose, pharynx, larynx, eyes and testicles. Skin lesions may occur as pale, anaesthetic macules, papules or erythematous infiltrated nodules. Neurological disturbances are manifested by nerve infiltration and thickening with anaesthesia, neuritis, paraesthesia and trophic ulcers. The disease is divided clinically and by laboratory tests into two overlapping types: lepromatous and tuberculoid. The lepromatous type (multibacillary or non-immune form) is progressive with nodular skin lesions, slow symmetric nerve involvement, numerous acid-fast bacilli in skin lesions and a negative lepromin skin test. The tuberculoid type (paucibacillary or immune form) is benign and non-progressive with localised skin lesions, asymmetric nerve involvement, few bacilli present in the lesions and a positive lepromin skin test.

CBS contributor Dobbs defends false leprosy claim after confrontation by CBS' Stahl
60 Minutes , CNN host and CBS Early Show edition Lou Dobbs Tonight of the false claims that "there were about 900 cases of leprosy [in the United States] for 40 years," and that "[t]here have been 7,000 in the past three years." The day after the 60 Minutes interview, during the May 7 edition of Lou Dobbs Tonight , Dobbs and Romans again defended the claims, with Romans attributing them to the late "Dr. Madeleine Cosman" (who did not have a medical degree but, rather, a Ph.D. in English and comparative literature ). In fact, according to the article that "in 40 years ... 900 people were afflicted." Cosman appears to have derived her false claim by misinterpreting a February 18, 2003, New York Times article At the end of the 60 Minutes profile, Stahl noted that "[w]hile we were talking to Dobbs, unbeknownst to us, he was talking to CBS News and has now joined The Early Show as a weekly commentator." Countries reporting the most cases in 1997 were Bangladesh, Brazil, India, Indonesia, Myanmar, and Nigeria (CDC, 1998). Agent and Vector : The acid-fast rod Mycobacterium leprae is transmitted probably via the respiratory route through prolonged exposure in childhood. Incubation: Usually 2-5 years; and up to 20 years. Clinical Findings and Treatment Signs and Symptoms : There are two basic types of leprosy: lepromatous leprosy (LL) and tuberculoid leprosy (TL). Either of these may be classified as borderline (borderline lepromatous or BL and borderline tuberculoid or BT). Patients who fall between the two basic types may be classified as borderline borderline (BB); and patients with vaguely defined (often early) lesions may be classified as indeterminate (I). Most initial infections involve few symptoms and spontaneous recovery is common, with a minority of patients developing clinical disease. Initial lesions tend to be vaguely defined hypopigmented or erythematous macules, i.e., indeterminate.
  • Lepromatous leprosy Tuberculoid leprosy is less malignant than LL and there is no or less deficiency in cell mediated immunity. Skin lesions are anesthetic and include well-demarcated macules and plaques that range from less than one cm to large enough to cover most of a body area. TL lesions may heal (centrally) spontaneously, leaving variously pigmented areas. Asymmetrical peripheral nerve involvement is common and leads to gradual loss of sensation, especially in the hands, feet, and face. Cutaneous nerves may be enlarged and/or tender.

THE MORNING NEWS addthis_pub = 'nwaonline'; There are no outbreaks of tuberculosis or leprosy in Northwest Arkansas, according to the Arkansas Department of Health, which released a statement Friday to correct what it called erroneous reports repeated all day to local, state and federal government officials.
"They (the reports of outbreaks) are not true," said Dr. Jim Phillips, branch chief of infectious disease with the state health department. "A lot of misinformation" has been circulated about both diseases in Benton, Washington and Sebastian counties, he said.
Calls were fielded by the Arkansas Health Department, the U.S. Center for Disease Control and 3rd District Rep. John Boozman's office, said Phillips. Also taking calls were the Springdale mayor's office and members of the state legislature.
The Health Department debunked reports claiming 300-plus cases of tuberculosis have occurred in the three counties. The number of cases statewide during 2007 was 106, the department said, with 21 cases were reported in Northwest Arkansas. No additional outbreaks are reported, Phillips said.

In 1978, NIAID awarded two contracts for the purpose of fractionating Mycobacterium leprae -infected armadillo organs to derive sizable quantities of M. leprae for use by the leprosy research community. Dr. Brennan's program at National Jewish Hospital was awarded one of the two contracts. Upon Dr. Brennan's transfer to Colorado State University in 1980, the work moved there. Thus, we have had this contract since 1979 through five subsequent recompetitions. Leprosy is a chronic, communicable disease caused by Mycobacterium leprae
  • The clinical spectrum of disease reflects the following:
    • bacterial proliferation and accumulation at the site of infection
    • the immunological response
    • peripheral neuritis
  • Peripheral nerves, skin and mucous membranes are affected- M. leprae only grows on the colder surfaces of the body
  • Leprosy is rarely an immediate cause of death
Incidence
  • Leprosy occurs predominantly in South-East Asia, but also in Africa and the Americas
  • There were about 1 million active cases world-wide in 1997, however, about 2 million people have irreversible deformities due leprosy
  • Each year about 600 000 people are diagnosed as having leprosy
  • The incidence is 2:1 males to females, although in Africa, it is 1:1
Incubation Period
  • Leprosy can occur in a few weeks following exposure, however, cases developing 30 years after exposure have been described
Transmission
  • M. leprae is not highly infectious
  • Transmission is more common among household contacts
  • Transmission is believed to occur through inhalation of infectious organisms
  • Transmission through insect bites and inoculation through broken (or intact) skin have not been excluded
  • Infectious people are thought to shed the organism from the nasal mucous membranes, especially if there is ulceration
