Skeletal weakness leads to fractures with minor or inapparent trauma, particularly in the thoracic and lumbar spine, wrist, and hip. Acute or chronic back pain is common. Diagnosis is by dual-energy x‑ray absorptiometry. Prevention and treatment involve Ca and vitamin D supplements, exercises to maximize bone and muscle strength and minimize the risk of falls, and drug therapy to preserve bone mass or stimulate new bone formation. Pathophysiology Normally, bone formation and resorption are closely coupled. Osteoblasts (cells that make the organic matrix of bone and then mineralize bone) and osteoclasts (cells that resorb bone) are regulated by parathyroid hormone (PTH), calcitonin, estrogen, vitamin D, various cytokines, and other local factors such as prostaglandins.

Although osteoporosis has no cure, several types of medications are available to reduce the rate of bone loss, increase bone density, and reduce the number of fractures. In general, they work in two main ways: they lessen bone break down (anti-resorptive agents) or they stimulate the formation of new bone (anabolic agents). Experts recommend that these medications be taken continuously in order to gain their full benefits. Generally, bone loss resumes if the medications are stopped. Most of the medications used to treat osteoporosis are also used to help prevent it. They include:
    • Calcium and Vitamin D
    • Hormone Replacement Therapy (Estrogens and/or Progestins)
    • Calcitonin
    • Bisphosphonates

    Osteoporosis may lead to tooth loss because the density of the bone supporting the teeth may be decreased. Researchers have suggested that a link between osteoporosis and bone loss in the jaw. Studies suggest that osteoporosis may lead to tooth loss because the density of the bone that supports the teeth may be decreased, which means the teeth no longer have a solid foundation. However, hormone replacement therapy may offer some protection. One study published in the June 2007 Journal of Periodontology (JOP) examined 1,256 postmenopausal women and looked for a potential association between periodontal bacteria and bone loss in the oral cavity. The study results showed that women with periodontal bacteria in their mouths were also more likely to have bone loss in the oral cavity, which can lead to tooth loss if not treated. A follow-up study of 106 postmenopausal women over more than 10 years concluded they could significantly reduce tooth loss by controlling their periodontal disease.

      Symptoms
      Many people confuse osteoporosis with arthritis and believe they can wait for symptoms such as swelling and joint pain to occur before seeing a doctor. However, the mechanisms that cause arthritis are entirely different from those in osteoporosis. Osteoporosis usually becomes quite advanced before symptoms appear. All too often, osteoporosis becomes apparent in dramatic fashion: a fracture of a vertebra (backbone), hip, forearm, or any bony site if sufficient bone mass is lost. These fractures frequently occur after apparently minor trauma, such as bending over, lifting, jumping, or falling from the standing position. Pain, disfigurement, and debilitation are common in the latter stages of the disease. Early spinal compression fractures may go undetected for a long time, but after a large percentage of calcium has been lost, the vertebrae in the spine start to collapse, gradually causing a stooped posture called kyphosis

    OSTEOPOROSIS:  Nutritional Causes, Prevention and Therapies
    Many individuals rely on extra amounts of Calcium + Vitamin D when trying to treat and/or prevent
    osteopenia (early stages of bone loss) or osteoporosis, and when unsuccessful, they generally resort
    to using drugs specifically formulated to stop the progression of this disease.
    Long-term effects of many of these medications are still unclear since increased bone mass doesn't
    always translate into increased bone strength.  On the other hand, when analyzing individual reasons
    for developing osteoporosis, it becomes clear that most factors can be resolved through a change in
    lifestyle, or through individually -tailored nutritional supplementation.
    Without the addition of weight-bearing exercise however, no program for the treatment or prevention
    of osteoporosis can be considered complete.  Climbing stairs, running, lifting weights - and to some extent walking - are all among those exercises considered beneficial in maintaining bone density, in contrast to activities such as swimming, which are less helpful for osteoporosis.
