    Introduction: Panic disorder
Panic disorder: Panic disorder causes episodes of irrational fear and panic called "panic attacks". In addition to the mental symptoms of fear , panic, and terror, these attacks often lead to physical symptoms such as chest pain difficulty breathing rapid pulse , and various other symptoms . These symptoms can be so severe that many first-time sufferers believe they are having a heart attack or are dying, and go to a hospital emergency room for diagnosis. A single panic attack does not necessarily mean a diagnosis of panic disorder, but multiple episodes usually does. Once an attack can occur, people can get into a vicious cycle of so-called "anticipatory anxiety", where the fear of having an attack leads to increased anxiety. In some cases, people start to avoid places or events that trigger an attack, leading to phobias or agoraphobia
Panic disorder: Panic disorder is very different from everyday anxiety. More than 3 million American adults have, or will have, panic disorder at some time in their lives. Most frequently, it starts in young adulthood. Usually, it does not go away by itself. But with proper treatment, people with panic disorder can be helped. Management strategies Treatment of panic disorder Panic disorder is a disabling condition that is common in patients in primary care settings. Diagnosis may be difficult because symptoms such as chest pain and shortness of breath also are associated with potentially serious conditions. However, proper diagnosis and treatment with medications and/or skilled therapy may restore a better quality of life. Patients with panic disorder typically have panic attacks, with rapid onset of the symptoms listed in Table 1 and a persistent concern about having an attack. Attacks occur suddenly and typically last more than 10 minutes (although the length of attacks is variable). They can occur one to several times per week, usually unpredictably, and may interfere with the patient's normal activities and work. Although panic disorder often is chronic, the frequency of attacks and associated symptoms (e.g., depression, avoidant behavior) may wax and wane. Panic disorder, as defined by the Diagnostic and Statistical Manual of Mental Disorders, 4th ed. (DSM-IV), affects 1 to 3 percent of the general population at some point in their lives. These patients, however, use health care resources to a disproportionately high extent. Psychiatric case-finding studies of patients presenting to emergency departments with chest pain found that 17 to 25 percent of these patients also met the criteria for panic disorder. In a large multi-center study of primary care practices, the prevalence of panic disorder ranged from 1 to 6 percent across study sites.

Panic symptoms arrive "out of the blue," and usually include shortness of breath, heart palpitations, and stomach upset. During a panic attack you may fear that you are having a heart attack, going crazy, or losing control. Sometimes, fear of having a panic attack leads people to stay home or avoid crowded situations. This is called agoraphobia and can lead to a very restricted lifestyle. Treatment for panic disorder includes anxiety management skills, followed by guided practice at experiencing panic-like symptoms without fear. Contents
Symptoms
Panic disorder sufferers usually have a series of intense episodes of extreme anxiety , known as panic attacks . These events may last from several minutes to hours, and may vary in intensity and specific symptoms of panic over the duration (i.e. rapid heartbeat, psychological experience of uncontrollable fear, etc.). Some individuals deal with these events on a regular basis—sometimes daily or weekly. The outward symptoms of a panic attack often cause negative social experiences (i.e. embarrassment, social stigma, ostracization, etc.). As a result, as many as 35% of all individuals with panic disorder also have agoraphobia citation needed
Occurrence
Panic disorder is a serious health problem but can be treated. It is estimated that 1.7 percent of the adult American population alone has panic disorder. It typically strikes in young adulthood ; roughly half of all people who have panic disorder develop the condition before age 24, though some sources say that the majority of young people affected for the first time are between the ages of 25 and 30. Women are twice as likely as men to develop panic disorder.

Panic Disorder
How a Panic Disorder Often Starts Because a panic attack is such an uncomfortable experience, a person learns to be hypervigilant to or oversensitive to even the smallest internal sensations (e.g., change in heart rate), including those triggered by medication changes or cardiac or other medical problems. A vicious cycle can get going. When a person notices an internal sensation, their sympathetic nervous system (SNS) activates, creating more sensations, more SNS arousal, and so on, causing a positive feedback loop that sometimes culminates in a panic attack. This is analogous to holding a microphone next to a loudspeaker. An initial signal is picked up by the microphone, amplified by the system, coming out the loudspeaker with more intensity, getting further amplified by the system, etc. So...
  • A panic attack is extremely unpleasant and frightening. This cascading of physiological and cognitive events reinforces the learning that certain internal sensations are something to be feared. Because of the learned hypervigilance and fear of internal sensations, the individual might also react phobically to sensations caused by medications and by medication withdrawal.
