  • Bacterial: Pathogenic organisms can gain access and stimulate inflammation. Chemical: caused by irritating or vesicant solutions. Mechanical: physical trauma from the skin puncture and movement of the cannula into the vein during insertion; any subsequent manipulation and movement of the cannula; clotting; or excessively large cannula. PHLEBITIS AND THROMBOSIS
Basically it means a clot in the vein. As the venous system is divided in deep veins and superficial veins one can have clot in each of the systems. Only very rarely clotting can occur in both of the systems in the same time. Superficial vein thrombosis or SVT is also called superficial thrombophlebitis/phlebitis. The "itis" denotes that the vein is engulfed in an inflamatory process: the skin around the vein is read and painful. Swelling can be present as well. Unfortunately, many physicians treat this condition with antibiotics. This is completely unnecessary as in most of cases there is no bacterial infection. The main cause for the condition are varicose veins. Blood stagnates in those venous pools and will clot easily especially during inactivity. Injecting varicose veins can lead to superficial phlebitis as well. In the absence of varicose veins one should look for other reasons such as malignancy elsewhere in the body, gout, Buerger's disease. or an inherited tendency to clot. ( See FAQ The treatment of the condition is simple: evacuation of clot if the patient comes early to the physician, local compression with ace bandages or surgical stockings, and ambulation. The only medication to be given is ibuprofen (Motrin/Advil) 5-7 days that will ease the pain and calm the inflamatory reaction. Occasionally SVT can be associated with DVT. Only in these cases is anticoagulation treatment necessary. If the underling condition for the superficial phlebitis was varicose veins after the inflamatory process has cooled off, surgery for the varicose veins is indicated.

The term phlebitis refers to an inflammation of a vein, usually in the leg, frequently accompanied by blood clots that adhere to the wall of the vein. When the affected vein is close to the surface, the condition is called superficial phlebitis. Risk Factors Risk factors for any type of phlebitis include recent surgery or childbirth, varicose veins , inactivity, sitting for long periods (such as on a long airplane ride), and smoking. Prolonged placement of intravenous catheters can also cause phlebitis, possibly requiring antibiotic treatment. The use of progestins (synthetic progesterones, but not natural progesterone ) will increase the likelihood of deep vein thrombophlebitis by 3 to 4 times.
Symptoms of superficial phlebitis include pain, swelling, redness, and warmth around the affected vein. The vein feels hard to the touch because of the clotted blood.
Deep vein thrombosis is more difficult to diagnose. It can occur without any symptoms until the clot reaches the

Introduction
"Thrombo" means "clot." Phlebitis is inflammation of a vein. Thrombophlebitis (throm-bo-fluh-BI-tis) occurs when a blood clot causes inflammation in one or more of your veins, typically in your legs. On rare occasions, thrombophlebitis (often shortened to "phlebitis") can affect veins in your arms or neck. The affected vein may be near the surface of your skin, causing superficial thrombophlebitis, or deep within a muscle, causing deep vein thrombosis (DVT). A clot in a deep vein increases your risk of serious health problems, including a dislodged clot (embolism) traveling to your lungs and blocking a pulmonary artery (pulmonary embolism). Thrombophlebitis often is caused by prolonged inactivity, such as from sitting during a long period of travel in an airplane or automobile or from lengthy bed rest after surgery. Such inactivity decreases blood flow through your veins and may cause a clot to form. Paralysis, certain types of cancer and use of the hormone estrogen also may lead to thrombophlebitis. An inherited tendency for blood clots places you at higher risk of thrombophlebitis. You can use self-care methods to ease pain and reduce your risk of clots. Various treatments, including medications and surgery, also are available for thrombophlebitis.

inflammation of the wall of a vein. Phlebitis may result from the infection of tissues adjacent to the vein, or it may result from trauma or from a surgical operation or childbirth. A long period of bed rest and an attendant lack of blood circulation may also cause phlebitis. Varicose veins, obesity, and atherosclerosis are other predisposing factors. In many cases the cause of phlebitis is not known. Phlebitis can last for years, and if it continues for a long time, the inner lining of the inflamed vein becomes irritated to the point that various elements in the blood are deposited there, forming a blood clot. This condition is known as thrombophlebitis Phlebitis usually occurs in one of the superficial veins of the lower leg. The condition is more serious when it occurs in a more deeply situated blood vessel, since if a blood clot develops there and then breaks away and begins to circulate in the bloodstream, it can cause a serious circulatory obstruction. The indications of phlebitis include localized pain, swelling, redness, and heat over the inflamed vein. Examination may reveal a tender, cordlike mass under the skin at the site. If phlebitis affects a superficial vein, the condition is relatively innocuous and can be treated by analgesics and bed rest until the inflammation disappears, at which time mild exercise should be taken. In severe or serious cases of phlebitis, anticoagulants are administered to prevent the formation of blood clots.

      What Is Thrombophlebitis?
      Thrombophlebitis occurs mostly in the legs.  It's a circulatory problem that develops when a blood clot slows the circulation in a vein, either right under the skin or deeper in the leg. The name gives you a hint about what it is: "thrombo" means clot, and "phlebitis" means a vein with inflammation, or the swelling and irritation that happens as the result of an injury. With thrombophlebitis (pronounced thrahm-bow-fleh-bye-tis), a blocked vein in the leg becomes swollen, irritated, and even hard to the touch. Most cases involving the shallow leg veins begin to resolve by themselves in a week or two.  But in rare occasions, these blocked veins can lead to infection and tissue damage from the loss of healthy circulation.  When the deeper veins in the leg are involved, there are greater risks. A portion of the clot can break off and enter the bloodstream, travel far from the injury site and cause serious problems. If the clot reaches the heart or lungs and blocks circulation there, it can even cause death. In the more serious cases, people with thrombophlebitis need to be treated with drugs to "melt" the clot, lessen the swelling, and treat any infection that may develop.

Superficial thrombophlebitis most often affects the superficial veins in the legs but may also affect superficial veins in the groin or in the arms. Superficial thrombophlebitis in the arms usually results from having an intravenous catheter. Often, thrombophlebitis occurs in people with varicose veins. However, most people with varicose veins do not develop thrombophlebitis. Even a slight injury can cause a varicose vein to become inflamed. Unlike deep vein thrombosis, which causes very little inflammation, superficial thrombophlebitis involves a sudden (acute) inflammatory reaction that causes the thrombus (blood clot) to adhere firmly to the vein wall and lessens the likelihood that it will break loose. Unlike deep veins, superficial veins have no surrounding muscles to squeeze and dislodge a thrombus. For these reasons, superficial thrombophlebitis rarely causes embolism.

Phlebitis is an inflammation of a vein. Thrombosis is the formation of a blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, blood vessel is called thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. Symptoms of phlebitis may include:
  • a hard and cord-like vein that is very sensitive to pressure or touch a surrounding area that is red and warm to the touch a slight fever or swelling of the extremity
The primary danger in thrombophlebitis is that the clot may move and travel to a vital organ, such as the lung, causing serious damage. If you have symptoms of thrombophlebitis or deep vein thrombosis your healthcare provider may order a doppler ultrasound examination (a test that uses sound waves to locate clots); a venogram in which dye is injected into a vein, usually in the foot. (X-rays are then taken to visualize the circulation and any blockages that may be present.) Lung scans may be ordered if there is concern that blood clots have traveled to the lungs. Thrombosis is treated with anticoagulant drugs, such as heparin or coumadin. These drugs inhibit the formation of blood clots.

When a vein is inflamed the blood in it is apt to form a clot, or thrombus, which, if loosened and displaced from its original position, may be carried as an embolus towards the heart and there be arrested; or it may pass through the cavities of the heart into the lungs, there to lodge and to give rise to alarming symptoms. If the thrombus is formed in the inflamed vein of a pile it may pass as an embolus (see HAEMORRHOIDS) into the liver . If an embolus is carried through the left side of the heart it may enter the large vessels at the root of the neck and reach the brain , giving rise to serious cerebral disturbance or to a fatal paralysis . The thrombus may be formed in gout and rheumatism , or in consequence of stagnation of the bloodcurrent due to slowing of the circulation in various wasting diseases. When a thrombus forms, absolute rest in the recumbent posture is to be strictly enjoined; the great danger is the displacement of the clot.     This is serious enough that you should see a doctor today. You are describing a condition called phlebitis, which simply means inflammation of a vein. While it sounds like you have superficial phlebitisthe involvement of a vein near the surfacewhich is rarely serious, we worry about a related condition called thrombophlebitis in which one or more blood clots causes pain and may block blood flow. In the worst-case scenario, deep vein thrombophlebitis (DVT) may involve a dislodged blood clot that may travel and block blood flow to the lungs; this potentially fatal condition is called a pulmonary embolus. It is possible for people with superficial phlebitis to also get deep vein thrombophlebitis, so a medical evaluation is necessary. Other risk factors include: a sedentary lifestyle (not getting enough exercise) or prolonged inactivity for any reason (including sitting in a car or on a plane for many hours); obesity; smoking; medical conditions that increase blood clotting; injury to the arms or legs; hormone replacement therapy; birth control pills; and pregnancy.
