Poliomyelitis (polio) Poliomyelitis is a highly contagious infection caused by poliovirus, which is transmitted from person to person through exposure to fecal material or respiratory secretions containing the virus. The incubation period ranges from nine to twelve days. Most poliovirus infections are asymptomatic. Initial symptoms, when they occur, are similar to those of other viral infections and may include fever, headache, muscle aches, malaise, nausea, vomiting, and sore throat. In roughly one in a thousand cases, poliovirus attacks the spinal cord or brainstem, leading to paralysis in various parts of the body, most often the legs. There has been a recent resurgence of polio in west and central Africa, chiefly Nigeria, related in part to a temporary interruption of immunization programs by local authorities in that country. Cases have also been reported from Benin, Botswana, Burkina Faso, Cameroon, Central African Republic, Chad, Ghana, Guinea, Ivory Coast, Mali, Niger, Sudan, and Togo. Poliovirus continues to be transmitted in the Indian subcontinent and in certain countries in the Middle East, particularly Egypt and Afghanistan. All children should receive four doses of inactivated polio vaccine at ages 2 months, 4 months, 6-18 months, and 4-6 years. An

M illions of Americans had taken the Salk and Sabin anti-polio vaccines by 1962, and an era of fear ended. "Seeing polio go away was like witnessing a major miracle," said Malcolm C. The virus inflamed nerves in the brain and spinal cord, causing paralysis of the muscles in the chest, leg or arms in severe cases. In 1950, 33,300 people were stricken. Lacking a vaccine, the nation tried to halt the spread of polio by closing public pools and parks. Parents ordered their children not to drink from public water fountains. Schools canceled graduations as a precaution. The March of Dimes raised money for a cure. Few people who saw them can forget the organization's poster children adorable tots with braces and crutches who posed in fund-raising pictures. For all its pain and grief, polio's era of fear provided knowledge that is useful today and would have been invaluable back in the '50s. For example:

But beneath this tranquil scene, parents faced a great fear-the dreaded poliomyelitis, or polio as it is commonly known. The disease had killed over thirteen hundred Americans (a large percentage were children) and crippled more than eighteen thousand more in the year 1954 alone. On April 12, 1955, American received the much-welcomed news that Dr. Jonas Salk had developed a vaccine against the frightening disease. Immediately, the federal government implemented a plan to have the vaccine produced by six licensed pharmaceutical companies and distributed to children throughout the country. Within one year, the deaths attributed to polio declined by 50 per cent, and this downward trend continues to the present when polio has been totally eradicated in most of the world. Living With Post-Polio Syndrome
Decades after surviving polio, many individuals are faced with the pain, fatigue and muscle weakness of post-polio syndrome (PPS). PPS is believed to affect 20-40% of the 640,000 polio survivors in the United States, according to the Post-Polio Task Force . People who had more severe cases of polio generally have more severe cases of PPS, as well. There is no cure for PPS, but it is seldom a life-threatening condition. PPS typically affects polio survivors 10 to 40 years after an initial polio attack. In the course of recovery from polio, the body compensates for the loss of nerve cells by sprouting extra nerve endings to restore function to muscles. It is believed that these "axonal sprouts" eventually fail after years of use, resulting in loss of muscle strength, pain and fatigue. In essence, the muscle fiber loses its ability to contract in an efficient manner. This may eventually lead to joint pain and skeletal deformities. But PPS is a "very slowly progressing condition marked by long periods of stability," according to the National Institute of Neurological Disorders and Stroke For example, patients may realize that in years past they could walk long distances, but are now fatigued by walking in the grocery story. Or they have pain or weakness in an area of the body they thought was unaffected by polio. However, not every polio survivor who has aches and pains or experiences fatigue has post-polio syndrome.
