Postpartum Depression: A Guide for Front-Line Health and Social Service Providers

Stewart This new guide on postpartum depression (PPD) is both practical and evidence-based, and includes the best and most current studies to date on PPD as well as practical experience from the field. Postpartum Depression was specifically developed to meet the needs of front-line health and social service providers who work with women during pregnancy and the postpartum period. It provides a unique Canadian perspective and covers diversity issues in more depth than existing resources. "Postpartum depression is a common, but frequently unrecognized, devastating mood disorder," says Kathryn Leopold, M.D., assistant professor of obstetrics at Albany Medical Center in New York. Postpartum depression (developed within six weeks of delivery) is severe and long-lasting, with symptoms including:
  • Anxiety, sadness or despair (constant mental basketcase and bitchy attitude)
  • Feelings of worthlessness or guilt, especially failure at motherhood (you will be blamed for everything she does wrong)
  • Loss of interest in usually pleasurable activities (sex stops, constant nagging starts)
  • Difficulty concentrating or making decisions (similar to mentally retarded child)
  • Fatigue (did nothing the whole day, yet always tired)
  • Changes in appetite or sleep (becomes fat, then obese)
  • Recurrent thoughts of death or suicide (completely mentally insane) The point here is that you don't want women's problems to become your problems. 67% of "women of independent minds" agree that they are mentally messed up. And that's before they give birth and have Postpartum depression on top of their existing mental problems. Truly scary stuff.

If it takes you longer to feel better, you may have postpartum depression. Postpartum depression can make you feel restless, anxious, fatigued and worthless. Some new moms worry they will hurt themselves or their babies. Unlike the " baby blues ," postpartum depression does not go away quickly. Very rarely, new moms develop something even more serious. They may stop eating, have trouble sleeping and become frantic or paranoid. Women with this condition usually need to be hospitalized.

    Postpartum depression is a serious disorder where the hormonal changes following childbirth, combined with psychosocial stresses such as sleep deprivation, may disable some women with an apparent underlying vulnerability. If you think you have it, tell your health care provider. Medicine and talk therapy can help you get well.

    About 10% of all women who have recently given birth develop postpartum depression. It usually begins after discharge from the hospital. It can be distinguished from the baby blues in that the women with the baby blues are able to function normally, and feel better by the time their baby is 2 weeks old.

    What is depression? Depression can be described as feeling sad, blue, unhappy, miserable, or down in the dumps. Most of us feel this way at one time or another for short periods. But true clinical depression is a mood disorder in which feelings of sadness, loss, anger, or frustration interfere with everyday life for an extended time. Depression can be mild, moderate, or severe. The degree of depression, which your doctor can determine, influences how you are treated. Depression after pregnancy is called postpartum depression or peripartum depression. After pregnancy, hormonal changes in a woman's body may trigger symptoms of depression. During pregnancy, the amount of two female hormones, estrogen and progesterone, in a woman's body increases greatly. In the first 24 hours after childbirth, the amount of these hormones rapidly drops back down to their normal non-pregnant levels. Researchers think the fast change in hormone levels may lead to depression, just as smaller changes in hormones can affect a woman's moods before she gets her menstrual period. Occasionally, levels of thyroid hormones may also drop after giving birth. The thyroid is a small gland in the neck that helps to regulate your metabolism (how your body uses and stores energy from food). Low thyroid levels can cause symptoms of depression including depressed mood, decreased interest in things, irritability, fatigue, difficulty concentrating, sleep problems, and weight gain. A simple blood test can tell if this condition is causing a woman's depression. If so, thyroid medicine can be prescribed by a doctor.
