Section 3: Aetiology and risk factors Retinal vein occlusion is due to thrombosis of retinal veins (central, hemi or branch) . Established cardiovascular risk factors are the predominant medical associations for both central and branch vein occlusions and are summarised below and include differentiation by age and ethnic groups. ( See table 1 Strength of evidence Hypertension This is the predominant risk factor with up to 64% of patients having hypertension ( Table 1 ) in the older age group (more than 50 years). This is more prevalent in BRVO than CRVO. A new diagnosis or uncontrolled hypertension is a common finding. Inadequately controlled hypertension is associated with recurrence of RVO in the same eye or fellow eye involvement. Diabetes mellitus Diabetes mellitus ( table 1 ) is associated with retinal vein occlusion. This may be due to an increase of other cardiovascular risk factors (e.g. 70% of type II diabetics are hypertensive). Glaucoma Current evidence suggests an association between central retinal vein occlusion and glaucoma. One study suggests that BRVO is associated with glaucoma Thrombophilia Factor V Leiden and other such abnormalities have been implicated in RVO but their aetiology is unclear. Deficiency or abnormal activity of various factors have been identified in patients with RVO including protein S,C, anti-thrombin 3, activated protein C resistance, hyperhomocysteinaemia and the presence of lupus anticoagulant or anti-cardiolipin antibody deficiency.

Question: Swoolen Lymphs on both sides, What they are?
bitta - Fri Jan 26, 2007 10:27 pm Hi All, I am a 44 year old male. About 5 months ago went to see my GP for an uncomforable feeling in my throat and night sweats. He ordered CT Scan. although there was nothing in my throat but they noticed about 3-4 swoolen Lymph nodes in my chest. One of them about 3 CM-all clustered at the same place. I had large Lymph nodes in my groin (left and right) At first I was told that I have a Lymphoma . and to make sure what type is it they did a needle biopsy. Result came out negative. Then my doctor ordered a second needle biopsy for Lymphoma TB (I previously had Abdomen TB treated 25 Years ago), Blood tests. All were Negative.

Iritis is the inflammation of the iris. If this is associated with inflammation of the choroid, it is called as uveitis (pronounced as you-v-i-tis). Many times retina also gets infected, termed as chorio-retinitis (Pronounced as ko-rio -retinitis) Causes Infections like Tuberculosis, Leprosy, Herpes virus infection, AIDS, Toxoplasmosis etc. Non- infectious causes like eye injury, Sarcoidoisis, Ankylosing Spondylitis, Juvenile Rheumatoid Arthritis etc. How do I know it is Iritis? Besides redness, there will be pain in the eye, watering, photophobia (inability to withstand day-light), decreased vision . This tells you that it is not likely to be just Conjunctivitis. To distinguish it from glaucoma, you will have to go to an eye-doctor and get your eye examined. He will take the eye pressure, examine you on the slit-lamp and then will make the diagnosis.
Management
Tests are required to know the cause so, treatment can be given accordingly. Blood tests, skin tests, X-ray Chest may be done.
Treatment
Steroid eye drops Any inflammation in the eye will cause scarring, which will hamper vision. To avoid this, the inflammation has to be urgently controlled.

A role of social and occupational factors in the development of chronic forms of respiratory sarcoidoisis and its relapses The social status and the presence of occupational and household hazards were analyzed in 134 patients with torpid chronic and undulating course of sarcoidosis lasting 3 years or more to estimate the significance of the above factors when the course of sarcoidosis was poor. The fact that 162 cases of hazards (metals, building materials, wood, textile, and paper dust, smoking, chemical substances, contact with patients with tuberculosis) occurred in 113 patients and 49 of the 134 patients with chronic forms of the disease were exposed to psychoemotional factors suggests that dust substances and chemical elements of different kinds, frequent stressful situations, and the X-ray signs of fibrosis of sarcoidosis-induced changes due to the absence of timely examinations should be regarded as a risk factors of the torpid chronic course of respiratory sarcoidosis (just at the stage of its primary detection).

Cancer cells divide rapidly, and drugs that interfere with folate metabolism are used to treat cancer. Methotrexate is a drug often used to treat cancer because it limits the activity of enzymes that need folate. Unfortunatley, methotrexate can be toxic (45-48), producing side effects such as inflammation in the digestive tract that make it difficult to eat normally. Leucovorin is a form of folate that can help "rescue" or reverse the toxic effects of methotrexate (49). It is not known whether folic acid supplements can help control the side effects of methotrexate without decreasing its effectiveness in chemotherapy (50-51). It is important for anyone receiving methotrexate to follow a medical doctor's advice on the use of folic acid supplements. mboxCreate('fa-widgettest');

Causes
Causes of infertility can be found in about 90% of infertility cases, but despite extensive tests, about 10% of couples will never know why they cannot conceive. Between 10% and 30% of cases of infertility have more than one cause. Male or female infertility each account for about 30% to 40% of cases. In men, sperm defects (their quality and quantity) are usually responsible. Female infertility is more complex.
Pelvic Inflammatory Disease
Pelvic inflammatory disease (PID) is the major cause of infertility worldwide. PID comprises a variety of infections caused by different bacteria that affect the reproductive organs, appendix, and parts of the intestine that lie in the pelvic area. The sites of infection most often implicated in infertility are in the fallopian tubes, a specific condition referred to as salpingitis Causes of PID.

What is sarcoidosis?
Sarcoidosis is an inflammatory disease characterised by the presence of non-caseating granulomas. A granuloma is a collection of inflammatory cells. They may occur in any organ or tissue, but the lungs, eyes and skin are most frequently affected. In most instances, the granulomas will heal with or without treatment. However, if the granulomas do not resolve and the inflammation persists, then scarring (or fibrosis) occurs leading to damage, or rarely failure of the organ involved. Who does it affect?
Sarcoidosis is a relatively common condition, which is estimated to affect approximately 1 in 2000 people worldwide. It occurs in all races, but is most often seen in people of Afro-Caribbean, Scandinavian, German, Irish, Peurto Rican and Asian decent. Sarcoidosis affects both sexes and, although it can occur at any age, it most frequently occurs in people aged 20-40years. What causes it?
How will it affect me?
How is it diagnosed?
Making a diagnosis can be difficult because the most of the symptoms can occur in a variety of other diseases and are not specific to sarcoidosis. It is therefore possible that it may occasionally be mistaken for other diseases, and vice versa. Although typical combinations of general complaints often raise the possibility of sarcoidosis, the diagnosis is usually confirmed following examination and investigations.
