        This chapter focuses primarily on men; Chapter 46 discusses vaginitis, mucopurulent cervicitis, and PID.

        Any person who engages in unprotected sexual activity is at risk for acquiring an STD, so screening is sound preventive medicine. Although the optimal frequency of STD screening visits is not well established, we recommend annual visits. In high-risk groups, such as sexually active adolescent females, testing for STDs should be offered twice a year. Indications for more frequent testing include a new partner and a history of sexually acquired infections.

        Patient History. A detailed sexual history is necessary, although time may be limited in a screening setting because of the extensive agenda. The physician should inquire about any history of sexual intercourse (frequency, last occurrence), gender preference, contraceptive or condom use, number of sexual partners within past 30 to 60 days and over patient's lifetime, and route of penetration (i.e., penile-vaginal, penile-anal, oral-genital). The normal review of systems should include asking about any symptoms of pain, discharge, dysuria, pruritus, or skin rashes. Female patients should describe their menstrual cycle, including timing of last menstrual period and symptoms of dysmenorrhea, amenorrhea, and spotting.

    Anal Sex Carries More Risks Of STDs
    Anal sex, also known as anal intercourse, is the term used for any sexual activity involving the anus, anal cavity, sphincter valve and/or rectum, but it’s more particularly used for erect penis insertion into the rectum through the anus. Anal sex is practiced by both heterosexual and homosexual partners. Anal sex without condom can be an effective way of passing on Sexually Transmissible Diseases (STDs). Anal penetration carries more risks than vaginal intercourse, oral sex, and outercourse. The reason is that rectum and colon are not self-lubricating like the vagina and the delicate colorectal tissue can get damaged more easily due to insertion and friction. Anal penetration can result in physical injury (anal rupture) because the colorectal passage curves are neither strongly muscled nor padded. Colorectal function includes absorption of fluid into the blood stream, providing an efficient entry point for STDs and an easy barrier to cross through even small tears in the intestinal lining.     Sexually transmitted diseases (STDs) are diseases that can be transmitted through sexual contact with another person. They can affect both men and women. There are often no symptoms.
    Protecting Yourself
    The best way to protect yourself against STDs is to limit your number of sexual partners. Condoms, when used properly, have been shown to be about 99% effective against the transmission of STDs. Using a dental dam or cellophane wrap when performing oral sex on a woman, while commonly suggested, has not been shown conclusively to protect against the spread of STDs as well as the condom does.
    The Diseases
    STDs are usually lumped into one big group of nasty diseases, and little is said about each STD individually. Here we provide an explanation of the most common STDs.
    Chlamydia
    Chlamydia is a bacterial infection that causes urethritis (inflammation of the urethra causing burning, pain, and discharge) in men and PID (pelvic inflammatory disease) in women. PID can lead to infertility. Chlamydia is treated with antibiotics.
    Gonorrhea
    Gonorrhea is a bacterial infection that causes urethritis in men and PID (pelvic inflammatory disease) in women. PID can lead to infertility. Chlamydia is treated with antibiotics.

In recent years, there has been a large increase in the number of sexually transmitted infections (STIs) in the UK. The number of people diagnosed with chlamydia, the most common STI, tripled between 1995 and 2005.
    Symptoms
    The following symptoms could indicate that you have an STI:
    • an unusual discharge from the vagina, penis or anus bleeding after sex or between periods sores, blisters, warts, rashes, irritation or itching near the genitals or anus

    What are sexually transmitted diseases (STDs)?
    Sexually transmitted diseases (STDs) are infectious diseases transmitted through sexual contact. They are among the most common infectious diseases - with the United States having the highest rates of STDs in the industrialized world.
    Prevention of STDs:
    The surest way to prevent contracting an STD is to abstain from any type of sexual intercourse. However, if you decide to become sexually active, or are currently sexually active, there are several precautionary measures you can follow, recommended by the National Institute of Allergy and Infectious Diseases, to help reduce your risk of developing a sexually transmitted disease. These include:
    • have a mutually monogamous sexual relationship with an uninfected partner use (consistently and correctly) a male condom use sterile needles if injecting intravenous drugs decrease susceptibility to HIV infections by preventing and controlling other STDs delay having sexual relationships as long as possible - the younger a person is when they begin to have sex for the first time, the more susceptible they become to developing an STD
