This is the single area where our National Resource Centre development has not kept up to the pace of the other developing departments but that will change very soon.
We started in April 2003 with a plan to 'Train the Trainer' but after a couple of months we were advised the plan would not work in its current format and that a review was needed.
This was complicated somewhat when we found that another venture in the marketplace to start Speech and Language Therapy was being confused and linked to us and that we had a direct involvement to it. This of course was not the case.

Communication difficulties may occur in the areas of language (the content and format of what we say and the way we say it), speech (the sounds we use and how we produce them), voice and fluency (stuttering/stammering). Since starting with Operation Cleft my aim has been to determine a service delivary model that would be appropriate for patients we will be seeing in Bangladesh. I have been in contact with a range of cleft palate specialists both nationally and internationally who have experience in working in developing countries and have provided with me a wide range of resources as I launch into this project. I have also been to the Royal Childrens Hospital and the Monash Medical Centre to observe their multidisciplinary cleft palate clinics. Unfortunately there is no clear cut answer and what is appropriate for the people of Bangladesh is likely to differ from what has been done in other countries. I have been in touch with speech pathologist in Sri Lanka who work with the Great Ormond Street Project (a UK initiative) which runs a very efficient cleft management team, and the speech pathologist in Chennai, India at the Sri Ramachandra Hospital who conduct training for community health workers for cleft palate management and run a cleft clinic. I will be going to see both of these projects in my upcoming trip to South East Asia.

    Categories: Childhood Development My youngest child is almost 2 1/2, and still doesn't speak many distinct words. Generally I am not that concerned about delayed speech, as I have known many "late talkers" make up the difference and match their peers very quickly. So when my pediatrician suggested testing and therapy at his 2 yr check up, I hesitated a bit. I didn't want to over-react to a little speech delay and add more chaos to our lives. However, my son would get extremely frustrated that we couldn't understand him. His reaction was to scream, and scream a lot! So I decided to go ahead and seek help for him, hoping we could develop some communication we could both understand. I first had his hearing tested, and he has no problems there. Then I had to decide if I wanted to seek therapy, and if so, where to get it. I decided to use our local early intervention office, Springfield Infant and Toddler Services. It's really been a lot of help, and fun for my son. A worker comes to our house once a week (or as many times/month as we agree to) and works one-on-one with him.
