  • Every year, over 1,000 people sustain a Spinal Cord Injury (SCI) and there are thought to be 40,000 people living with SCI in the UK alone

What is it? The spinal cord runs through a chain of boney rings known as the vertebrae. This vertebral column protects the spinal cord that works like a telephone cable sending messages of feeling and sensation to the brain which converts them into responses such as movement. When the spinal cord is injured these messages are interrupted or cannot get through at all. Depending on the extent of damage to the spinal cord, a person will be either partially or completely paralysed from the point of damage (lesion) downwards.
Causes
The spinal cord is injured usually through trauma (injury) to the delicate fibres of the spinal cord. Common causes of spinal cord injury include road traffic accidents, diving accidents and sports injuries. But it is not always trauma that damages the spinal cord. Viruses and viral infections, cysts and growths on the cord can all cause permanent damage.
Types of injury Paraplegia
- resulting from a broken back, is paralysis from the chest or waist downwards, with little or no movement or feeling in the lower limbs and lower part of the trunk.

    A spinal cord injury refers to any trauma which results in damage to the spinal cord. The type of symptoms produced will vary according to the severity of the damage and the location along the spinal cord (Spinal Cord Injuries Australia 2004). The spinal cord controls both motor (muscle) and sensory nerves. As such, damage to the spinal cord may result in loss or reduction of muscle movement and strength, and/or loss or reduction of sensation (an ability to feel touch, pressure, temperature, etc) on the skin surface (Spinal Cord Injuries Australia 2004).
    Symptoms and characteristics
    The symptoms will vary greatly depending on the location and severity of the injury. The higher up the spine the trauma, the more of the body will be affected below that level. Spinal cord injuries in the lower back (lumbar and sacral levels) will result in symptoms occurring in the legs and usually the pelvis. Injuries in the central back (thoracic level) will also involve trunk muscles and organs, such as bladder and bowel control (Tintinalli et al 2004). Trauma to the neck (cervical level) will also involve reduced function in the arms and shoulders. Upper cervical trauma, near the base of the skull, may also result in difficulties with breathing and body temperature regulation (Tintinalli et al 2004).

SPINAL CORD INJURY Injury to the Spinal Cord can occur due to trauma to any part of the spine from the cranium to the upper lumbar region where the spinal cord terminates. Trauma to the spinal cord is commonly associated with damage to the spine itself. Often, a fracture of the spine occurs in relation to the injured central nervous system tissue. The area where the spinal cord is injured dictates the level of function, as dysfunction develops below that specific level. Spinal cord injuries are classified as either incomplete or complete. Incomplete injuries show preservation of some function below the injured level. Complete injuries are characterized by a total lack of both motor and sensory function in motor-sensory and electrical modalities below the level of the injury. Symptoms: Spinal cord injury presents with severe pain, limited mobility, or paralysis after a specific accident or trauma to the spine. Diagnosis: Patients with spinal cord injury need to be thoroughly examined on admission to assess any preservation of function, including peri-anal and rectal examinations and testing of all reflexes and motor and sensory functions. Sematosensory evoked potentials (SSEP) are also a possible adjunctive test. They examine the ability of the spinal cord to transmit impulses by stimulating an area of the leg or arm with a weak electrical current, and determining whether or not this stimulus can be detected over the corresponding part of the brain. Wherever possible, radiographic studies (x-rays, CT scan) need to be performed in a timely manner. An

    Approximately 250,000 to 400,000 people in the United States have spinal cord injuries. Each year, approximately 11,000 people sustain new spinal cord injuries. Most of these injuries are caused from auto and sports accidents, falls, and industrial mishaps. An estimated 60 percent of these individuals are 30 years old or younger, with the majority of them being men.
    The spinal cord is the major bundle of nerves that carries nerve impulses to and from the brain to the rest of the body. The spinal cord runs from the base of the brain to your waist. The brain and the spinal cord constitute the Central Nervous System. Motor and sensory nerves outside the central nervous system constitute the Peripheral Nervous System, and another diffuse system of nerves that control involuntary functions such as blood pressure and temperature regulation are the Sympathetic and Parasympathetic Nervous Systems.
    Spinal cord injuries fall into two categories, complete and incomplete. A complete injury means that there is no function below the level of the injury, no sensation and no voluntary movement. As a result both sides of the body are affected equally. An incomplete injury means that there is some functioning below the primary level of the injury. Individuals with an incomplete injury may be able to move one limb more than another, may be able to feel parts of the body that cannot be moved, or may have more functioning on one side of the body than the other.

There is no cure for SCI but stem cell research has shown some signs of being useful in the future.

That's the question Stacy Fritz hopes to answer through a new project funded by the S.C. Spinal Cord Injury Research Fund. Fritz, a clinical assistant professor in the Department of Exercise Science, plans to work with 16 clients during the next two years, most of whom have exhausted their regular insurance benefits for physical therapy related to their injuries. "Most people with incomplete spinal cord injuries have something sparedsome ability to stand for a short time or to sit up and move from bed to chair," Fritz said. "What would happen if we bombard them with intensive therapy for several hours a day? Will it help them regain some of what was lost?" To join the program, clients must be able, with assistance, to stand for two minutes and walk 10 feet. Fritz and her team concentrate on improving gait, balance, and mobility by working with the clients for three hours a day for 10 days. The sessions, held in the Arnold School of Public Health's new research center, put participants on a treadmill for hours, suspended in place by straps and simulating a walking motion to improve their sensations of gait and balance. The therapy also works on improving simple transition movementsfrom bed to chair, for examplethat might make it easier for participants and their caregivers to perform such tasks after the therapy concludes.
