    Vitiligo is a dermatological condition characterized by the appearance of white patches of skin on different parts of the body as a result of the destruction of the cells that make pigment (melanocytes). This may vary from one or two white spots on the skin to large areas of depigmentation. Vitiligo is not contagious. It seems to occur more often among people who have certain autoimmune diseases. For some people, although not for everyone, the depigmentation is progressive.

    Return to ITM Online TREATMENT OF VITILIGO WITH CHINESE HERBS by Subhuti Dharmananda, Ph.D., Director, Institute for Traditional Medicine, Portland, Oregon The primary treatment for vitiligo for the past several decades has been PUVA (psoralen and ultra-violet A). Psoralen is a chemical compound derived from herbs, especially from Psoralea cordyfolia PUVA therapy is not very satisfactory except for small patches; while some people respond well, the majority attain only partial pigmentation and are not happy with the results (1). The therapy is disruptive, in that it may need to be applied repeatedly (with individual office visits) for many weeks. Over-the-counter, self-applied psoralens have been developed as an alternative; while more convenient, this doesn't necessarily improve the outcomes. The PUVA method is most effective when vitiligo is limited to only one or two clearly defined patches, and less useful when there are multiple patches.

    Vitiligo appears to be an acquired condition and may appear at any age. There is an increased incidence in some families. Vitiligo is more noticeable in darker skinned people because of the contrast of white patches against dark skin. The cause of vitiligo is unknown, but autoimmunity may be a factor. This condition affects about 1% of the U.S. population.

He was prescribed imiquimod three times weekly. After using the medication for 4 months, the patient complained of erythema at the sites of application without improvement of the warts. The patient stopped using imiquimod and returned to see us 2 months later. On examination, there were depigmented patches in the areas of application (Fig. 1).
    Discussion
    ]. Imiquimod treatment associated pigmentary changes have been reported and are listed as possible side effects on the package insert. Of the 1257 adverse reports made from 1997 to November 2003 in association with imiquimod use, 68 (5.4%) were reports of pigmentary changes: 43 reported occurrences of depigmentation, 7 of vitiligo, 1 of hypopigmentation, and 17 of hyperpigmentation [ Imiquimod does not directly kill the human papilloma virus (HPV), but rather causes regression by enhancing the host's innate immune response and by promoting a local response via the induction of cytokines [

    Vitiligo All people except those born albinos have functioning skin pigment cells called melanocytes. These are cells or “cytes” with the root melas coming from the Greek word for the color black. Each individual with functioning melanocytes, whatever his arbitrarily assigned or geographically presumed race, has within himself, an assortment of pigment colors that, along with the characteristic response of those color-bearing melanocytes to sunlight, determines the appearance of one’s skin color at a given time. We know mostly about melanocytes from their dread connection with the category of cancers known as melanomas. But a surprise best-seller that came out in mid-November 2007 may change that: Lee Thomas’ Turning White: A Memoir of Change (Momentum Books, $14.95, ISBN 13: 978-1-879094-81-9). Lee Thomas is an African-American Fox network TV entertainment reporter who has vitiligo: a disease in which some of his melanocytes are either destroyed or underperform in their role of producing skin or hair color or pigment. This has dramatically changed his facial appearance (and the appearance of his hands) into one of mixed patches of black and white skin. His forthright and stirring book is very likely to raise public awareness and demand for library information on vitiligo.
