    • Repigmentation in Vitiligo: PUVA vs. Narrowband UVB
      Narrowband UVB was safer and more effective than PUVA for vitiligo therapy. Phototherapy employing sunlight and plants that contain natural psoralens was first used for vitiligo thousands of years ago, and it continues to be a first-line treatment today. Investigators conducted a randomized, double-blind trial to compare two of the most popular forms of phototherapy for vitiligo in 50 patients with nonsegmental vitiligo who received either PUVA or narrowband UVB (NB-UVB) therapy (25 patients in each group). Comment: This study clearly demonstrates that for vitiligo repigmentation, NB-UVB is safer and more effective than PUVA. Other randomized, controlled trials have shown at least some efficacy with targeted phototherapy, topical and systemic steroids, topical calcineurin inhibitors, and calcipotriene combined with PUVA. A number of reports show success with surgical repigmentation procedures, as well. Although we are making progress in treating this vexing disease (especially with calcineurin inhibitors and phototherapy), we still have a long way to go. At best, 75% of patients respond, and in most instances, this response is only partial. A breakthrough in the management of this disease would be greatly welcomed by both patients and doctors. An accompanying editorial provides a nice evidence-based review of existing therapies for vitiligo.

    Vitiligo Introduction
    Brochure Vitiligo also known as leucoderma, which simply means white (leuco) skin (derma), i.e. a disorder where the skin loses its normal coloration. It is also medically known as achromia, which means loss of color.
    Basically, Vitiligo can hardly be called as a disease but a skin-disorder that has more social than medical significance, especially amongst the dark skinned people. Due to destruction of the melanin (pigment) cells due to lesser known processes (largely what is called as an auto-immune disorder) the normal skin starts loosing pigments from various parts of the skin, in a varying speed and extent. Our clinical experience based on the treatment of over 4500 cases, suggests that there is a strong genetic factor in the background of most cases, especially those who have extensive vitiligo or those who have vitiligo affecting the finger-tips, toes, lips or the genitals. The indication of strong genetic factor is observed in the form of family history of one or more of the auto-immune diseases such as vitiligo, diabetes, hypothyroid, alopecia areata, cancer, rheumatoid arthritis or allergies. The vitiligo sufferers are observed all over the world, including the white skin communities. However, epidemiologically most cases are recorded in India (8.8%) and Mexico. Estimated 1-2% of the Americans have vitiligo as per the survey made by the American Academy of Dermatology. Males and females are affected equally, inclusive of children age group. It may begin at any age. Childhood vitiligo is not uncommon.

Vitiligo is a common, often inherited disorder characterized by areas of well-defined, milky white skin. People with vitiligo may have eye abnormalities and also have a higher incidence of thyroid disease, diabetes mellitus, and pernicious anemia. Vitiligo affects about 1-2% of the world's population. I must tell you first that I am not a medical doctor (M.D.), I am a basic scientist (Ph.D.), so I cannot offer you specific medical advice about treating your vitiligo. Hopefully I can still provide you with some helpful information through this website. Finding a dermatologist who is knowledgeable about vitiligo and its treatment is not always easy. Many vitiligo patients and parents of children with vitiligo share referrals on VSIG . There are a few other places to look on the web to find a dermatologist that might be able to help you. The American Academy of Dermatology website has a link called " Find a Dermatologist ". The National Vitiligo Foundation website has a link called " Physicians ", which includes an international listing.
